Gout is a type of arthritis that usually starts in the big toe and can affect other joints, such as knees, elbows, wrists, and ankles. The disease is characterised by bouts of intense pain in the affected joints. It may also cause joints to swell, stiffen, and deform.

The Four Phases of Gout

Gout is a progressive disease that, if not checked, worsens over time. The progression follows four main phases:

Phase 1: Asymptomatic Gout

This is also called the hyperuricemia phase. It is characterised by high levels of urate in the blood—the root of gout. If the uric acid problem is detected at this early stage, it can be mitigated to avoid gout altogether. Unfortunately, hyperuricemia alone does not exhibit any symptoms. So, this phase goes mostly unnoticed.

Phase 2: Acute Gout

Phase 3: Intercritical Gout

Intercritical or interval gout is a relatively calm period between the first gout attack and the next. This interval could be weeks, months, or even years. During this time, the urate crystals quietly build up in the joints until they reach another critical point.

Phase 4: Chronic Tophaceous Gout

The gout progresses to its final stage and becomes tophaceous. The uric acid crystals accumulate heavily around the affected joint to form tophi. This is the most problematic phase of gout and can easily cause irreversible damage to joints.
